Membership Options
Basic Membership
Included annually:
- Yearly dental X-rays
- Full mouth series every 36 months
- Two periodic examinations per year
- Two routine dental cleanings per year*
- One fluoride treatment annually for children up to age 14
*Routine prophylaxis only. Advanced hygiene services such as scaling and root planing or full-mouth debridement are not included.
Periodontal Membership
Included annually:
- Yearly dental X-rays
- Full mouth series every 36 months
- Two periodic examinations per year
- Two routine dental cleanings per year
- Two periodontal maintenance visits per year

Important Plan Information
This plan is administered exclusively through the office of Lena D. Karkalas, DDS and is not dental insurance.
Limitations & Exclusions
Patients who choose to utilize dental insurance for treatment will be subject to their insurance plan’s rates, copays, deductibles, and limitations rather than the membership discount.
Services performed by specialists or providers outside our practice are not covered.
Dental procedures completed before or after an active membership period are not eligible.
Services covered through workers’ compensation, government programs, or other benefit sources are excluded.
Membership discounts cannot be combined with insurance benefits or other third-party payment programs.
Membership Policies
Membership automatically renews annually unless cancelled in writing.
Cancellation requests should be submitted at least 30 days before the renewal date.
Missed appointments without 48-hour notice may be subject to a $70 fee.
Membership may be terminated for repeated missed appointments, abuse of benefits, or failure to pay membership fees or treatment balances.
Membership fees are non-refundable once services have been utilized.
